I thought I knew a lot about carpal tunnel syndrome. Turns out I didn’t know diddle.

So I searched everywhere. Books, magazines, the internet. I was desperate for answers.

I got a lot of info on who gets it, how much it costs in loss productivity and a bunch of statistics I didn’t care about. There was also quite a bit on how to use a computer ergonomically.

There was also misinformation mixed in with truth. Many websites just copied other websites – misinformation and all.

There were some stretches. The most helpful thing I got was using ice.   In the evening I would dip my arms in alternating hot and cold 5 gallon buckets.

There had to be a better way.

Finally the breakthrough came. From a most unusual source. 

An article on the Internet. Members of a rowing team were developing numb hands. They found relief by rubbing above the wrist with their thumbs.

But my poor thumbs were already overworked.

Also they missed a vital part of what needed to be done.

Right away I was practically pain free

I’ve had this condition since ’94 and just lived with it. Hi Hilma,
I’m a graphic designer on the computer all the time. And this is where my wrist problems started.  Love all the advise that I’ve been doing with your programs. Tremendous help. I went to three doctors before I started your cousre.

1st Doctor put me on pain medicine, told me I had RA and referred me to a Rheumatology.

2nd Doctor (the Rheumatologist) said stay on the pain medicine (meloxicam) and also put me on pennsaid. Didn’t have RA. Went back, still had pain sent me to a specialist for more tests.

What is the Carpal Tunnel?

The carpal tunnel is like a ditch lined with wrist bones. Across the top of that ditch is a hard strap called the transverse carpal tunnel ligament.

Through that “tunnel” passes the median nerve and nine flexor tendons (described later).

Numbness Pattern for Median Nerve Median Nerve Splits – Yellow Portion Does Not Go Through the Carpal Tunnel.

If the median nerve is compressed you will have numbness or other sensations in the palm side of the thumb, first two fingers. and half of the ring finger. (Never the little finger)

That numbness is the primary symptom of Carpal Tunnel Syndrome (CTS).However, there could be other causes.

If it is carpal tunnel syndrome and it caused by things you have done with your hands either through your occupation or hobby or other things you do in life…

One is from repetitive use of your wrist and/or fingers. (or more importantly how you do the repetitive motions).  I’ll get to number 2 later.

When you move your fingers up and down, like in typing, that motion is actually caused by your forearm muscles. 

You can prove that by grasping your upper forearm with the opposite hand and feel what happens when your move your wrist and fingers.

The muscles on the palm side of the forearm are called flexor muscles. They are the ones that close your fingers. The ones on the back side of the forearm are the ones that open the fingers. They are called extensor muscles.

The forearm muscles attach to your fingers through long skinny tendons. Those tendons go through hollow tubes that are called tendon sheaths. And there is lubrication in those sheaths.

Those tendons are like cables. When you move your fingers up and down, up and down, the tendons move back and forth, back and forth in those tendon sheaths.

That lubrication is not motor oil. You know that in the engine in a car, those pistons move up and down. They can up and down all day. But if you run out of motor oil or there is a lot of dirt in it, the friction causes the engine to seize up. The motor is ruined.

Tip: If you use your hands and fingers for more than 20 minutes at a time without giving them at least a five minute break, you’re drying out the synovial fluid (the lubrication) in there. And that causes friction. And that causes tiny pieces of your tendon or your tendon sheath to chafe off.

And that accumulates gunk and irritation inside those tendon sheaths and they start to swell up.

Tip: When you’re working, have the back of your hand and the back of your forearm in a straight line. Why? When those tendons go around a corner it is like having a rope around a tree branch that you’re pulling back and forth.

When your tendons have to go around corners, you’re creating the possibility of more friction. more accumulation of gunk in the tendon sheath. All that causes swelling and less room in the carpal tunnel.

What a lot of doctors do – and they call it a conservative method – is prescribe anti-inflammatory drugs. 

While that decreases the the swelling in the short term – in the long run it makes your problem worse. Because it interferes with your natural healing process.

Drugs have NEVER EVER cured carpal tunnel syndrome.

Knowledgeable doctors know this. But a huge number of doctors apparently don’t.

Why drugs are bad for carpal tunnel syndrome
Not even counting potential side effects

Your body has these cells that are called macrophages that can ooze in and out of just about anything in your body.  They gobble up anything that doesn’t belong there: like dead cells, bacteria, even little pieces of metal. Then they transport them out of there.

But when you are taking drugs, the brain doesn’t get the message that there is a problem. It’s like having firemen in the firehouse never getting the message that there is a fire.

The Second Major Cause of Carpal Tunnel Syndrome…

Tight forearm muscles. And Imbalance in those muscles. Whether it’s from repetitive use of your hands or from gripping. Could be from gripping anything from weights, to jack hammers, to machinery, to mountain bikes, to pruning shears, or anything else.

You are using the muscles that close your fingers a lot more powerfully than the muscles that open the fingers. And that causes muscle imbalance and tight muscles in the forearm

When muscles are overused, muscle fibers can get stuck together. Or muscle cells can get stuck in contraction.

Either way, that shortens the forearm muscles on the flexor side (palm side).  And that shortening causes the wrist to curl forward, causing less room in the carpal tunnel.

What do doctors do about that?

They recommend that you wear a wrist splint at night. And that’s actually a good idea because that keeps the carpal tunnel open while your are sleeping so your hand isn’t flopping around in bed.

But that’s not curing anything. 

The real solution is to loosen up those tight forearm muscles.

If the wrist splint doesn’t make any difference, then your problem is coming from somewhere else.

I’ll explain that later when I talk about the “Dimmer Switch Analogy.”

The purpose of the surgery is to create more room in the carpal tunnel to take the pressure off the median nerve.

They are not fixing the nerve. They are taking the pressure off it.

If you could take the pressure off yourself, you wouldn’t need the surgery.

The Nerve Runs Under the Ligament

The reason you are born with the transverse carpal tunnel ligament is it provides leverage for gripping.

Practically everyone who has this surgery is left with a weaker grip.

Of those whose grip strength returns, it is because of scar tissue formation.

Surgeries of any kind carry the risk of serious infection. Or additional damage can be caused.

What a surgeon calls success and what a patient calls success are usually not the same. Most surgeons claim a 92% to 93% success rate – or 7 to 8 surgeries out of 100 fail for various reasons.

What a surgeon calls success is that the pressure is relieved off the median nerve, regardless of whatever new pain the patient feels or how long their time is off of work.

Surgery is not an instant magic bullet.

Let’s go back to Jim, the dirt bike racer.

How did I know he did not have carpal tunnel syndrome, after only a few minutes of discussion? How did I know, when the doctor who gave him those cortisone shots didn’t know?

How did I know where his problem WAS coming from?

He told me that when he put his hands down the numbness went away.

Put your arms up and hold them there for about 5 minutes. If you get more numbness in your hands, but when you put them down the numbness goes away…

That is caused by a muscle on the side of the chest that you almost never feel pain it. But that muscles is tight and pressing on your nerve. And you feel it in your hand. That brings us to…

“The Dimmer Switch Analogy”

I’m sure you are familiar with a dimmer switch. If you turn it up the lamp gets brighter. You turn it down the light gets dimmer. What it does is it controls the amount of electricity going through the switch.

Now lets say there were a series of dimmer switches and the lamp was dim. You could not tell by looking at the lamp which switch was dimming – or if multiple switches were a little bit dimmed.

The lamp couldn’t tell either. The only way to tell would be to examine each switch.

Well your nerves are a lot like electric wires. The nerves that run into your hand, come out of the lower neck vertebrae, go down the front of your neck, under your collar bone, under your arm pit, and down your arm.

And when you feel numbness in your fingers, you brain thinks that’s where the trouble is. Rubbing your fingers won’t help any more than rubbing the lamp.

There are several places in your body where tight muscles or muscle imbalance can pinch on nerves and mimic carpal tunnel syndrome.

Almost no one is told this, let alone told what to do about them.

Let me tell you about one of those places that is so overlooked and causes so much mischief.

There are muscles in the front of your neck that can cause all kinds of problems. But you almost never feel pain in them – which is why they are so overlooked. The shenanigans they can do varies widely. But among the nasty things they can do

They can pinch on the nerves that go into your shoulder and into your hand. That causes numbness in the hand and can even cause shoulder problems.
